We request a 24 hours’ notice for cancellations. Cancellations made prior to this window are rescheduled with no penalty. Cancellations made without 24 hours’ notice but prior to the start of the session incur a $50 late-cancellation fee. No-shows or cancellations made after the start of the session will incur the full fee.
For new clients first appointments, a no show or late cancellation will result in a full charge of the new client fee.
I understand that by signing this Agreement, I shall be the party responsible for the payment of fees, dues and any other charges payable to the Serene Aesthetics & Wellness Spa. I understand I must timely notify the Spa of address, phone, e-mail, or payment changes.
I understand that all of the following apply to any service at the Spa or concierge including but not limited to Massage Therapy, and future services:
(A) Payment: All services must be paid up front in full via cash, or credit card before session can begin. Services are non-transferrable and non-refundable.
(B) Cancellations: Must be done at least 24 hours prior to the scheduled appointment or appointment will be forfeited without refund. Three “no-shows” will result in cancellation of your session.
(C) Packages: All packages expire. See specific Package expiration dates associated with Package. Each session must be initialed by the member and the Instructor to show completion. Any package that is purchased is for that lesson type only, and cannot be transferred to another package type, or redeemed for a different service.
I acknowledge and understand that the services offered by Serene Aesthetics & Wellness Spa are not a substitute for regular medical care, proper diet, and other actions related to the maintenance of good health and that the services and facilities offered by the Serene Aesthetics & Wellness Spa are not those of a physician, mental treatment facility, or healthcare facility and should not be used as a substitute for the same.
I have read this consent and release before signing below, and I understand and agree to its terms.
A deposit is required for all Spa bookings. Full payment is due at the time of booking for all concierge services. Deposit fees are non-refundable. A 24-hour notice is required for rescheduling and cancellations. Appointments cancelled same day will result in forfeiture of all monies paid. Same day reschedules are considered same day cancellations.
